Your new role


The Care Coordinator will lead and coordinate the support services offered to NDIS eligible participants and disability clients in a remote location. This will also include the coordination of case management and brokerage services. To work with the Training Coordinator to co facilitate the Training of a capable workforce and to ensure the recruitment and onboarding process is undertaken in a timely manner and meets quality practices.

  • Provide professional direction and support to support staff for the provision of best practice to our participants.
  • Communicate promptly with After-hours services if required to ensure continuation of placement support outside of standard office hours.
  • Conduct regular and ad-hoc checks on placements ensuring compliance; ensure that any actions are completed within a reasonable and acceptable time frame.
  • Conduct regular staff reviews with support staff to proactively support staff and provide guidance/information, and motivation, identify areas of improvement and set achievable measurable goals to attain these.
  • Coordinating all aspects of the organisations Health applicant's recruitment and HR processes
  • Liaising with Support Workers and provide feedback
  • Supervision of support workers
  • Preparing and distributing rosters
  • Preparing weekly /monthly and quarterly reports for internal and external requirements
  • Support Afterhours requests

Skills and Experience required

Essential:

  • Computer literate including excellent skills in using Windows based software, Internet, Email.
  • Exceptional interpersonal skills inclusive of clear and accurate communication (both written and verbal) and influencing skills necessary to deal effectively with clients, teams and stakeholders.
  • Demonstrated experience in health care & support, child welfare services or related industries.
  • Possess a current First Aid and CPR Certificate.
  • Demonstrated ability to coordinate activities, to use innovative approaches to ensure staff, clients and key stakeholders benefit.
  • Exceptional ability to work with teams that includes managing stress, tension and potential critical care situations with a common sense / solution focused approach.
  • Experience working within a remote community setting
  • Ability to follow cultural protocols and communicate effectively with Aboriginal and Torres Strait Islander people
  • Must hold or be willing to obtain a Blue and Yellow (QLD).
  • Current QLD Open Drivers Licence.


Desirable:

  • Tertiary Qualifications in health care or community services
  • A minimum of 3 years work experience in health care or community services
  • Understanding and experience in Australian Indigenous protocols, practices and affairs.
