About the Department
The Health Independence Program (HIP) at SVHM consists of Complex Care Services (CCS), Subacute Ambulatory Services (SACS) and the inner North Melbourne Post-Acute Services based at North Richmond Community Health (NRCH). HIP offers a comprehensive range of specialist clinics, services and rehabilitation programs, providing rapid response health management, short-medium term care coordination and health education.

HIP enable clients to be the decision makers of their care, and support clients through self-management to achieve their healthcare goals. HIP consists of interdisciplinary teams based at NRCH and SVHM Fitzroy, and care is delivered to clients under a SVHM HIP Community Hub model. As HIP is developing, this role is currently based with the Complex Needs Aged Care Team at our NRCH. In future this role may be required to be delivered at/from different community Hubs. Clinical service delivery is provided to clients in their own home and local community.

About the Role
This position has two components:

Complex Care Physiotherapist: As a physiotherapist, you will provide assessment, intervention and management to achieve optimal functional recovery, encourage self-management and promote empowerment for clients to improve their health outcomes well being and quality of life.

Complex Aged Coordination: you will provide person-centred care coordination for clients living with complex aged care and healthcare needs, utilising discipline specific skills to support clients to improve their self-management skills.

Classifications for this position will range from Physiotherapist Grade 2 VB1 - VB4 ($83,361 - $95,825 per annum).

Duties

  • Direct clinical provision of high quality and evidence based physiotherapy specific assessment, intervention, education and management
  • Undertake specialist assessment of physical and functional status for clients with complex chronic and multiple conditions
  • Provision of clinical interventions including, but not limited, to mobility retraining, equipment prescription, falls prevention and make referrals for ongoing follow up/long term interventions as required
  • Undertake holistic comprehensive assessments in collaboration with the client using motivational interviewing and self-management approaches
  • Collaborative care planning with clients, carers and/or families in the context of their health, functional, psycho social, cultural, environmental and economic needs

Qualifications, Skills and Experience

  • Degree or Diploma in Physiotherapy
  • Eligible for registration/membership with AHPRA
  • Current Victorian Driver's License
  • Minimum 2 years post graduate physiotherapy experience collectively in the fields of acute, sub-acute, or community
  • Demonstrated sound knowledge of the application of care coordination practice frameworks and principles, in the context of the current health care environment
  • Demonstrated ability to work autonomously and as part of an interdisciplinary team
  • Demonstrated ability to effectively work with relevant service providers to achieve best outcomes for clients
