About Department
This position is part of the Health Independence Program (HIP). The intent of HIP is to deliver improved outcomes for clients and to support hospital demand management and flow, through delivering integrated and coordinated care to clients across the hospital and community interface, reducing duplication and fragmentation of services and ensuring client centred care planning.

SVHM HIP consists of integrated hub sites across several locations including in Fitzroy, Kew, North Richmond and Northcote.

HIP: Psychosocial stream (ALERT) sits within the SVHM Complex care Services, the key target groups for ALERT include patients with complex needs who fall under one or more of the key risk factors; substance use, mental health issues, homelessness, disability and complex aged care issues, family violence.

This position is based at St Vincent's Hospital Melbourne within the Emergency Department.

About the Role
The ED Care Coordinator through screening and recruitment in the ED is responsible for the timely and comprehensive assessment and care coordination of patients with complex needs to facilitate the allocation of hospital and community resources in an efficient and cost-effective manner.

This position will receive clinical feedback directly from the ED Clinical coordinator within ALERT and strong links with the ED department.

Classifications for this position will range from:

  • Occupational Therapist Grade 2 VF6 - VF9 ($81,328 - $93,490 per annum)
  • Physiotherapist Grade 2 VB1 - VB4 ($81,328 - $93,490 per annum)
  • Social Worker Grade 2 SC21 - SC24 ($81,328 - $93,490 per annum)
  • Registered Nurse Grade 4A YW7 - YW8 ($101,764 per annum).

Duties

  • Early identification of ALERT patients in the Emergency Department.
  • Maximize a patient's functional independence through assessment and treatment planning, implementation and evaluation.
  • Provide appropriate discipline specific clinical management, discharge planning & community liaison for all relevant patients.
  • Effective negotiation and advocacy with relevant service providers to ensure optimal and coordinated client care according to best practice principles
  • Ensuring all documentation including client records are completed and managed in a timely manner consistent with SVHM and Complex Needs policies and procedures, legislative and regulatory requirements

Qualifications, Skills and Experience

  • Minimum 3 years' experience in relevant acute setting
  • Eligible for registration/membership with AHPRA.
  • Experience with prioritizing patients and hospital demands
  • Demonstrated sound knowledge of the application of care coordination practice frameworks and principles, in the context of the current health care environment.
  • Proven ability to effectively manage and negotiate solutions to complex problems
  • Participation in roster which includes weekends, Public Holidays and afternoon shifts.
