About the Program
Rehab@Home is a 12 bed admitted (bed substitution) program for patients in a home-based setting. This service is integrated with the Rehabilitation Ward located at St George's. The service will provide interdisciplinary, 7 day a week rehabilitation services from a team including a Rehab Physician, Nursing, Allied Health and Pharmacy. Telephone after hours support will be provided, as required, by the St. Georges Hospital After Hours Coordinator.

About the Role
Grade 2 Physiotherapist are required on a casual basis to assist the team when rostered. The shifts are 6 hours in length and rostered on a day of the weekend. The Rehab@Home weekend Physiotherapist will work alongside a Rehabilitation Nurse and interdisciplinary AHA. A Rehab@Home Occupational Therapist will work on the alternate weekend day. This position is responsible for providing high quality clinical management of patients, in particular at their homes and via a telehealth model of care. You will be responsible for the development, implementation and delivery of a high quality Physiotherapy service to inpatients of SVHM. You will be supported by an allied health team leader and a subacute team leader of Physiotherapy.

Classifications for this position will range from VB1 - VB4 ($78,900 - $90,700 per annum).

Duties

  • Demonstrate sound clinical knowledge of Physiotherapy practice and principles in the sub-acute stream
  • Ongoing demonstration & evaluation of knowledge translation into practice, in the delivery of Physiotherapy to the inpatients of SVHM
  • Excellent communication, time management skills to maintain effective PT service, discharge planning and patient flow
  • Provide optimal Physiotherapy clinical management to all patients through the continuum of care by assessment, treatment, re-evaluation, documentation, team liaison and effective discharge planning
  • Actively participate in departmental clinical supervision program
  • Actively participate in the evaluation of the effectiveness of the Physiotherapy Rehab@Home service

Qualifications, Skills and Experience

  • Relevant clinical experience at Grade 2 level.
  • Current registration with AHPRA and eligibility for membership to the APA, as well as a current Victorian motor vehicle license are essential
  • Collaborative team work and a commitment to quality improvement initiatives is integral to the role as is working within a proactive learning model to guide professional development opportunities
