WE ARE LOOKING FOR A QUALIFIED AND EXPERIENCED DISABILITY SERVICE

OFFICER FOR IMMEDIATE START ACROSS THE SUNSHINE COAST AND

CABOOLTURE REGION

We ensure our team members have work/life balance and are well supported in fulfilling their roles.

We currently service clients between Gympie and Caboolture.

Our Requirements and job description:

  • A Diploma in Health and Social Care or similar
  • At least 2 years’ experience in similar role
  • Work holistically with clients to assess full needs and requirements of client. This includes needs for health, community and other daily activities
  • Ensure clients are working towards their NDIS goals and report on progress via progress reports.
  • Liaise with external stakeholders and external supports. These may include day programmes, community groups, allied health and government agencies (public guardian etc)
  • Assessing clients’ needs and develop training and support programs to support staff
  • Leading a team of Support Workers
  • Monitoring group home roster’s and ensuring staff have the relevant training required to work with each client.
  • Source relevant external training from allied health for clients with complex care needs
  • Facilitate team meetings where required
  • Participate in “On Call” after hours support and back-up
  • Provide a quality service for residents, providing best practice standards in support rights, interests and needs. Coordinating resident appointments, health and wellbeing supports ensuring all appointments and needs are met
  • Ensure that clients health is supported by regular Healthcare checks, and associated documentation
  • Work within restrictive practice legislation if required
  • Ensure service is delivered in line with the National Standards for Disability services, and other relevant legislation
  • Implement, contribute to review, follow and promote Home Caring policies, procedures, work instructions and Work Health and Safety guidelines and instructions.
