Correctional centres are responsible for the delivery of custodial services across Queensland. People in custody are supervised in either high or low security correctional centres. High security centres provide for the management of adult prisoners held in secure custody correctional centres across Queensland.

The Through Care Planning Coordinator is the primary contact for prisoners in relation to their offender plan and through care needs. This position will be responsible for the development of a progression plan and will act as a conduit between the prisoner and key stakeholders. These stakeholders will include Probation and Parole, the Parole Board and service providers. The Through Care Planning Coordinator will provide timely advice to the prisoner and key stakeholders.

In this role you will:
  • Undertake assessment interviews with prisoners assigned to caseload to identify needs and priorities
  • Work in conjunction with the prisoner and their case officer to develop a through care plan to address their rehabilitation and reintegration needs
  • Remain informed on prisoner progress and events through regular liaison with correctional officers and access to prisoner information
  • Encourage prisoner motivation through the use of a range of techniques and tools including motivational interviewing
  • Monitor prisoner progress and regularly review prisoner plans ensuring all rehabilitation and reintegration services are provided to the prisoner
  • Contribute to prisoner management processes by providing high level advice in relation to prisoner rehabilitation and reintegration issues
  • Ensure the timely provision of advice to courts, boards and other agencies and stakeholders regarding prisoner reintegration
  • Ensure reintegration needs of prisoner are met prior to release including preparation of individual release plans in consultation with Probation and Parole and relevant reintegration services
  • Promote continuous improvement and innovation in the delivery of reintegration services to prisoners
  • Participate in planning activities to ensure prisoners have timely access to appropriate reintegration services
  • Ensure maintenance, accuracy and appropriateness of records as required
  • Develop and maintain effective relationships with key stakeholders to assist with the reintegration of prisoners
  • Ensure continuity of service provision across custody and the community by liaising with Probation and Parole Officers to facilitate prisoner transition
  • Ensure appropriate community based assessments inform current practice
  • Participate in team meetings and other activities within the office
  • Ensure the safety and security of self and others
  • Ensure the security and confidentiality of QCS property and information.
  • Manage one's own time efficiently and effectively
  • Ensure one's own personal presentation and behaviour are in accordance with the Agency's Code of Conduct
  • Undertake any other task as requested by line manager
