About the Role

Deliver core welfare services as delegated by the welfare worker team leader inherent to the principles of the Australian Community Workers Association (ACWA) Code of Ethics to ensure patient focused outcomes.
Provide comprehensive welfare assessments of patients and family/carers to assist in the provision of timely and effective welfare interventions.
Facilitate patient access to appropriate government and community resources to ensure equity in patient outcomes

This includes:
Developing skill in the application and implementation of patient travel and accommodation schemes
Referral, liaison and negotiation with government and non-government financial support services
Develop a comprehensive knowledge of resources to ensure best patient outcome in relation to financial aid, access and referral options.
Demonstrated ability to undertake routine tasks, with developing ability to undertake more complex welfare service provision under the guidance of a more experienced practitioner.
