As Support Coordinator, you will be responsible for supporting people with disability to find the right services to improve their lifestyle, giving them choice and control over their services. The Support Coordinator is responsible for connecting people with a National Disability Insurance Scheme (NDIS) Support Plan to access a range of supports that align with the support items in their NDIS plan and suit their individual needs. As a Support Coordinator in Brisbane, you will provide varying levels of coordination and transition support for young people and adults, considering individuals’ circumstances and complexity levels.


We will provide you with a central office, computer, phone and access to pool cars, gratifying career, great pay rates and a friendly and collaborative environment.


To work in partnership with clients on improving and evaluating the delivery of services; Attract new clients to the service by promoting services and accepting requests for a support coordinator; To provide direction and supervision to staff to ensure our clinical practice meets the needs of our clients; Develop and maintain relationships with all stakeholders; Deliver quality services to clients in line with policies and procedures.



To be a productive member of the Total Support Services and provide a range of therapeutic services to clients; Provide crisis intervention services, conduct psychosocial assessments and make recommendations for care and intervention; Develop and review support plans to ensure they meet the needs of the clients; Provide supervision and direction to support workers to ensure they are working in line with the support plans; Maintain accurate records of involvement and contribute to Assessment reports; Attend care team meetings and provide written reports as required.

Communication and Team Work

Establish therapeutic relationships with clients and other stakeholders; Establish working relationships with medical, nursing and allied health staff, community clinicians and other service providers; Provide feedback and training to staff on the outcome of assessment and recommendations; Actively participate in client meetings to ensure effective communication and dissemination of information; Foster a culture of openness, respect, accountability and professionalism in the workplace; Represent the Total Support Services inter-agency meetings and other meetings as negotiated.

Client Management

Maintain client documentation, including Support plans completed annually or more frequently if required; Maintain file note for each contact with clients; Review client file notes, ensuring they are factual and appropriate; Provide advocacy for clients and negotiate to fund for clients where required; Coordinate supports to ensure services are provided in line with funding and individual support plans.


MSC1: A degree in Human Services/ Disability/ Social Work/ Health

MSC2: Current driver’s licence

MSC3: DCSI Screening Clearance


KSC1: Demonstrated skills in counselling both children and adults.

KSC2: Demonstrated knowledge of the evidence-based practice

KSC3: Extensive experience in Social Work practice & demonstrated ability and understanding of case management

KSC4: High quality written and oral communication skills & strong computer skills

KSC5: High level of effective communication with stakeholders


DSC1: Good working knowledge of child protection

DSC2: Good working knowledge of NDIS
