About the Department
This position is part of the Health Independence Programs (HIP). The intent of HIP is to deliver improved outcomes for clients and to support hospital demand management and flow, through delivering integrated and coordinated care to clients across the hospital and community interface, reducing duplication and fragmentation of services and ensuring client centred care planning.

SVHM HIP consists of integrated hub sites across several locations including in Fitzroy, Kew, North Richmond and Fairfield.

HIP at SVHM is delivered in partnership with North Richmond Community Health (NRCH).

This position will be part of the HIP Community Rehabilitation team. Community rehabilitation is currently provided from our centres at Kew and Fairfield.

The key aim of Community Rehabilitation is to help people who are recovering from traumatic injury or illness, are frail or chronically ill to regain optimal function and to allow people to maximise their independence.

The programs offer a supportive and dynamic team environment which focusses on patient-centred care.

About the Role
An exciting opportunity currently exists for a Grade 2 physiotherapist to join the health independence programs team. The Grade 2 physiotherapist will deliver high quality person-centred, goal-orientated and home and centre-based care within a supportive multidisciplinary team.

Predominantly a rehabilitation role, the position may rotate through Polio Services Victoria (PSV).

Polio Services Victoria (PSV) is a state-wide clinic established to provide expertise and support for people with polio and post polio syndrome and involves multidisciplinary clinics, hydrotherapy and gym groups

This position may be required to work at any of the HIP hub sites or programs. This will be at the discretion of the HIP manager and will take into consideration appropriate skills and training.

Classifications for this position will range from Physiotherapist Grade 2 VB1 - VB4 ($81,328 - $93,490 per annum).

Duties

  • Delivery of accountable client focussed, high quality physiotherapy services to all clients within your caseload
  • Communicate client issues effectively with other team members, with active contribution to multidisciplinary team meetings
  • Evaluate client treatment plans using appropriate methods
  • Work collaboratively with the multidisciplinary team
  • Develop, evaluate and adapt group programs as required
  • Contribution to service improvement projects

Skills and Experience

  • Current Physiotherapy Registration Bachelor of Physiotherapy or equivalent
  • Minimum of 4 years post-graduate experience in a relevant field of rehabilitation
  • A demonstrated knowledge and application of therapy practice and principles in the area of Rehabilitation
  • Evidence and commitment to ongoing professional development
  • Knowledge of and commitment to continuous improvement
  • Excellent leadership and interpersonal skills with people at all levels
  • Current Victorian Driver's Licence
