Service Coordinator required for Wagga and Temora.


Your new company
Be the primary point of contact for clients ensuring their care plans are developed and reviewed to meet their ongoing needs. Lead and support a team of Care workers to provide client-centred services in a cost effective way to deliver the most appropriate services within relevant frameworks.

Your new role
Duties to include but are not limited to:

  • Drive customer acquisition and revenue growth targets by uncovering new clients
  • Initial client assessment - developing effective client plans negotiate and develop care plans with the client, their families and carers, in line with relevant policies and guidelines
  • Ensure care plans are accurately entered into relevant systems
  • Effective handover of new clients to Allocations with minimal disruption to clients
  • Establish the client relationship to maximise a positive introduction to Australian Unity Service Delivery
  • Conduct annual review of care plans with the client, their families and carers in line with relevant policies and guidelines
  • Establish and maintain the client relationship to maximise service delivery satisfaction
  • Establish and maintain external stakeholder relationships to maximise service delivery for clients
  • Ongoing review and management of Client budget to ensure services are maintained within budget allocation
  • Liaise with Allocations to ensure a seamless experience for the client
  • Deal with Customer complaints in a timely and effective manner
  • Conduct well-being checks with clients


What you'll need to succeed
  • Demonstrated ability in case management including assessing, planning and monitoring client needs Demonstrated experience working with clients and carers in a community based organisation
  • Demonstrated experience leading a team and managing performance of employees
  • Effective time management skills
  • Decision making and problem solving skills
  • Conflict resolution skills
  • Ability to work autonomously
  • Understanding of funding and package options
  • Working with clients to achieve optimal funding outcomes
