Position Summary

We’re seeking an experienced Community Services Care Coordinator to join our Community Services Leadership Team across Brisbane South!

Overseeing a team of personal care staff, you’ll work to provide outstanding care to clients in your region, advocating individualised care that enhances and promotes their quality of life. In collaboration with your Care Manager you’ll empower your team of Lead Practitioner and Disability Support Workers through learning, support and leadership in order to advance the level of service to our clients.

Working closely with your Community Care Manager, you’ll ensure quality and compliance governance are achieved through robust processes that meet internal and external auditing standards.

You’ll ensure the sustainability and growth of the service through your diligence with community funding, care planning and community engagement. You’ll plan and implement care initiatives centred on quality improvements, driving positive changes within your services.

What you’ll need:
As an experienced Community services professional, you’ll hold considerable knowledge of community care with practical experience leading personal care teams within a NDIS & community setting. You’ll have a solid understanding of the industries quality standards, frameworks, funding and how these have shaped care delivery for services. You'll have:

Experience coordinating NDIS funding and service agreements for NDIS participants
Excellent written and verbal communication skills
Proven leadership, coaching and development skills
Sound experience in management of budgets & financials
Ability to routinely problem-solve complex clinical scenarios
Evidence of innovative practices, through quality or service improvements

In May 2020 it became mandatory to have the influenza vaccination to either enter or work in a Residential Aged Care Facility. This was a legal direction triggered by the COVID-19 Pandemic. Recently, Queensland Health advised that employees working in a residential aged care facility are no longer required to have an influenza vaccination. Whilst the influenza vaccination is currently not a requirement, it is highly likely the vaccination will be a seasonal requirement moving into future years. Desirable: Do you have full Australian working entitlements as an Australian Citizen, Australian Permanent Resident OR New Zealand Citizen?
