An exciting collaboration between The Wesley Hospital and Blue Care allows you to support patients through their post-acute recovery in the comfort of their own homes.

Blue Care is actively seeking applications from experienced and passionate Registered Nurses to join our Brisbane North Hospital in the Home Team.

The current opportunity would suit Registered Nurses with proven PICC maintenance and Wound VAC experience looking to restore their work life balance in a self-managed role.

Your role:

As part of a newly created Hospital in the Home initiative, we’re seeking highly qualified Registered Nurses to assist patients with their recovery through post-acute clinical care.

Now, more than ever, it is of upmost importance that patients get the best support post op, returning to home and focusing on their recovery. Working closely with the hospital medical team, you will support early patient discharge by assisting with the development and delivery of care plans to meet our patient’s needs.

As a Registered Nurse providing acute care, you’ll provide person-centred health care to our clients within their homes. The acute care service will be offered 7 days a week primarily between 8:00am and 8:00pm with after-hours organised on a rotational basis.

This position will have an after-hours on-call support requirement.

You’ll ensure a safe environment and apply best clinical practice to enhance and maintain the quality of our client’s lifestyle during their transition from an inpatient to an outpatient setting.

Your duties and responsibilities will include, but will not be limited to:

  • Clinical assessments, monitoring and updating care plans
  • IV therapy, cannulation, venepuncture, PICC, CVAD maintenance and complex wound care
  • Continence management, medication management & titration, & chronic disease management

You’ll work collaboratively with a multi-disciplinary team, supporting one another to deliver outcomes and identifying opportunities for improvements centred on client’s health and wellness within their home. With no two days ever the same, you’ll spend the majority of your day on the road, completing your scheduled community visits, interacting one on one with clients and their loved ones!

You’ll demonstrate:

You’ll be an experienced nurse who is driven to make a change in the lives of our elderly clients through enabling their independence and well-being. You’ll be confident working autonomously within a community care based setting and demonstrate:

  • Current unconditional (Division 1) nursing registration with AHPRA
  • Recent experience delivering acute nursing care services
  • An ability to apply mobile information technology devices in the delivery of care
  • Basic life support certification and a current driver’s license
  • Evidence of vaccination (or proof of non-susceptibility) to vaccine preventable diseases
  • Valid open Australian drivers licence

Our process includes undergoing a Federal Police check and some positions will require you to complete an online medical assessment. We pay for both of these and it’s an easy process. You also need full working rights for Australia.

Supporting our clients in the best way possible is important to us. To be employed by Blue Care, you will be required to complete the NDIS Worker Module prior to commencement and provide a copy of your certificate of completion.
