JOB DESCRIPTION

Provide competent core Social Work clinical services as a member of the multidisciplinary team to patients and to their families/carers across the acute wards as delegated, within Kilcoy Hospital, to optimise the patients' psychosocial outcomes, and to achieve maximum benefits from health care services. This position will be expected to cover all clinical areas as directed by Director of Social Work.

About the Role

Deliver competent social work clinical services across a variety of caseloads within an acute hospital setting in accordance with prescribed professional and ethical standards
Work cooperatively and effectively with patients, families and carers, members of the multidisciplinary teams, members of the social work team, and relevant government and non-government stakeholders utilising well developed communication and negotiation skills
Actively participate within the discipline of social work and the multidisciplinary clinical team's environment to ensure that social work services are integrated, efficient and equitable for patients, families and carers
Relevant to the position, participate in the ongoing education, implementation, monitoring and evaluation of safety and quality initiatives relevant to The National Safety and Quality Health Services Standards and the Australian Council on Healthcare Standards (ACHS) Evaluation and Quality Improvement Program (EQuIP).
