About the Service

Logan Family Recovery (LFR) is a unique residential rehabilitation program which makes it possible for parents to receive support while their young children stay with them. The units are located on the site of Logan House, Chambers Flat. The program is on a quiet, rural setting. Our specialist team includes drug and alcohol and family counsellors, a treatment facilitator, a registered nurse and other allied health professionals.

About the role

As the LFR Alcohol and other Drugs Counsellor Case Manager you will have a caseload of clients and work with parents and families to deliver the program. Reporting to the Team Leader – LFR, the role will work to ensure that residents’ needs are addressed in a holistic manner, to achieve optimal recovery and health outcomes. You will work with clients who have completed the residential rehabilitation program, providing face-to-face, phone and virtual AOD counselling and case management, along with facilitating relapse prevention groups within the rehabilitation program and in the community. You will also attend community stakeholder meetings where appropriate to facilitate referrals to relevant services for clients.

You will thrive in this opportunity to:

  • Provide high quality engagement, assessment, counselling and support services to consumers and their families (where relevant) using evidence informed practice and principles
  • Facilitate the intake process for the transition house in collaboration with other relevant service providers
  • Collaborate with existing service providers, including the development of referrals to relevant services in line with the transition goals
  • Promote ‘housekeeping’ activities that facilitate and support the day to day operational aspects of the program

This is a full-time ongoing position based at Chambers Flat. The salary range for is $72,361.12 to $77,854.04 annually.

What you will bring to the role

Your ability to working autonomously and collaboratively in a team will ensure success in the role. You will also demonstrate the following:

  • Tertiary qualifications in a related area health service
  • Applied knowledge in contemporary evidence based AOD counselling practice, case management and intake and assessment.
  • Highly developed communication and community engagement skills.
  • Experience in delivering education programs and facilitation groups (desirable).
  • Local area knowledge (desirable).
